This is a quick guide for getting the Old Habits achievement/trophy and also 100% game progress in AC Origins. Like many, I was stuck at 99% for a while so I know the feeling of having "completed everything" and still not seeing 100%. Below are some tips for what to do (and what not to do) to get over that last hump.
The description for Old Habits is simply "Complete all locations." That's pretty vague. Here is what they mean by Locations:

If you're already at 100%, congratulations! Otherwise, double-check your Ptolemy statues against an online guide. There are only 19 of them so it doesn't take long. I missed the one in Ineb-Hedjet Nome, probably because its icon was obscured by the Fast Travel location of Per Ousir.
Finally, here's the big secret to getting 100%. There are two "hidden" side missions that only show up on your map when you're right on top of them. Otherwise, they disappear.
The first is called "The Bride". Fast travel to Soknopaiou Nesos in Faiyum. Find the bride standing on a cliff just northwest of the Blacksmith marker on the map:
The other hidden side mission is "What's Yours is Mine". It can be found here in Giza:
People say that "Loose Cargo" is also hidden but I didn't have any problem finding that one on my own. It starts just north of the Zoiontegoi Shipwreck in Herekleion Nome with a Roman soldier trapped in a cage.
Another tip: From the "Quests" tab you can see how many quests you've completed. You can also scroll through their names and check for "The Bride", "What's Yours is Mine" and "Loose Cargo".
There's disagreement about how many quests make up the main game. I ended up with 130 including "Secrets of the First Pyramids" which I bought with reward points from Ubisoft Club. The Xbox website gave me credit for 100% after completing 129 quests. I guess it just counts to 129 and didn't realize that one of them was "Secrets of the First Pyramids". Ubisoft's site only registered 100% upon completing my 130th quest.
Update: When it comes to finding Hermit Locations, Senu is no help but holding D-Pad Up is. It's that thing that illuminates nearby loot with white triangle icons. Hermit Locations are indicated with a yellow triangle.
